Information Required to Apply

To become a member of Kindersley & District Co-operative Limited, applicants must be at least 16 years of age and provide the following mandatory information during the application process:

  • Full Name
  • Birthdate (required to meet by-law age requirements)
  • Mailing Address with Postal Code (for equity allocation and cheque mail-out)
  • Phone Number
  • Signature
  • Social Insurance Number (SIN) (see clarification below)

Without all required information, Kindersley & District Co-op is unable to process or sell a membership.

Why a SIN Is Required?

Your Social Insurance Number (SIN) is required under the Income Tax Act for tax reporting purposes and to issue a T4A slip when applicable.

Please note: Kindersley & District Co-operative Limited is required by law to have this information on file

Collecting your SIN does not violate the Personal Information Protection and Electronic Documents Act (PIPEDA)

Your SIN will not be used for any other purpose

If your equity allocation exceeds $100.00 and your purchases are for personal use, providing your SIN allows you to recover withholding tax held by the Canada Revenue Agency (CRA) when filing your annual personal income taxes.

This means you may benefit not only from Co-op’s equity and cash back program, but also receive additional funds back through CRA.
